人工股骨头置换在老年不稳定型股骨转子间骨折治疗中的临床应用

摘要

目的 探讨人工股骨头置换在老年不稳定型股骨转子间骨折治疗中的临床价值及安全性.方法 对2005年1月至2011年3月期间收治的46例老年不稳定型股骨转子骨折患者,根据患者意愿采取不同术式治疗.29例采取人工股骨头置换术(人工股骨头手术组),17例采取动力髋螺钉内固定术(内固定术组).观察术中出血、手术时间、卧床时间、术后并发症等指标,术后随访进行Harris评分.结果 两组患者术中出血量及手术时间比较差异无统计学意义(P>0.05);行人工股骨头置换术患者平均术后卧床时间明显短于内固定术患者,并发症发生少于内固定术患者,两组比较差异有统计学意义(P 0. 05 ); the average time on bed in femoral head surgery group was obviously shorter than that of fixation group, so as in complications, and the difference between these two groups was significant ( P < 0. 05 ). Harris scores were evaluated after 1 month, 3 months and 6 months, there was significant difference between these two groups ( P <0.05 ). Conclusion For elderly patients with unstable intertrochanteric fracture, the treatment with hemiarthroplasty has fewer complications, shorter time in bed, good recovery of hip function, it is significantly better than those of internal fixation, hence it is an effective treatment method for these patients.
