超声诊断不同分型多囊卵巢综合征的表现差异对比

Objective To compare the difference in performances of ultrasound in diagnosis of different types of polycystic ovary syndrome.Methods 200 patients with polycystic ovary syndrome treated in this hospital during May 2014 to May 2016 were selected for this study, they were divided into obese group (BMI≥25 kg/m2, n=100) and non-obese group (BMI< 25 kg/m2, n=100) according to body mass inde, patients in these two groups were given with ultrasound examination by color Doppler ultrasonography (GE company, U.S.A).The correlation between ultrasound performances and endocrine indices had been statistically analyzed.Results The ovarian volume in patients of obese group was significantly larger than that of patients in non-obese group (P<0.05).The ovarian volume and HOMA-IR showed significantly positive correlation (r=0.46, P<0.05), uterine artery RI and PRL concentration showed significantly negative correlation (r=-0.57, P<0.05).The number of ovarian follicles, ovarian volume and FT concentration in non-obese group showed significantly positive correlation (r=0.30,0.55, P<0.05).The number of ovarian follicles and LH/FSH showed significant positive correlation (r=0.33, P<0.05).Conclusion Obese women with polycystic ovary syndrome have larger ovarian volume than those of non-obese women, ultrasonography and endocrine indicators have some relevance.%目的 对比不同分型多囊卵巢综合征的超声表现差异.方法 选取2014年5月至2016年5月收治的多囊卵巢综合征患者200例,依据体重指数将这些患者分为肥胖组(BMI≥25 kg/m2,n=100)和非肥胖组(BMI<25 kg/m2,n=100)两组,采用美国GE公司生产的彩色多普勒超声诊断仪对两组患者进行超声检查,然后对其超声表现、内分泌指标及其二者的相关性进行统计分析.结果 肥胖组患者的卵巢体积显著大于非肥胖组(P<0.05),卵巢体积与胰岛素抵抗指数(HOMA-IR)呈显著的正相关关系(r=0.46,P<0.05),子宫动脉阻力指数(RI)与泌乳素(PRL)浓度呈显著的负相关关系(r=-0.57,P<0.05);非肥胖组患者的卵巢卵泡数量、卵巢体积均与游离睾酮(FT)浓度均呈显著的正相关关系(r=0.30、0.55,P<0.05),卵巢卵泡数量与黄体生成素与卵泡刺激素的比值(LH/FSH)呈显著的正相关关系(r=0.33,P<0.05).结论 肥胖型较非肥胖型多囊卵巢综合征具有较大的卵巢体积,超声表现与内分泌指标具有一定的相关性.
