脑胶质瘤的转移

摘要

目的：探讨颅内肿瘤的转移方式、方法。方法：选择5例在肿瘤切除后又于远隔部位肿瘤复发的胶质细胞瘤病例，分析其CT、MRI影像特点，手术所见及手术后病理检验结果，判断新生肿瘤与原发病之间的关系及转移特点。结果：2例室管膜瘤均向颅外手术切口附近皮下转移，3例髓母细胞瘤患者于原发肿瘤切除4a后，分别于大脑额颞极表面侧脑室壁及脊髓表面转移。结论：颅内胶质细胞瘤可发生转移，其中室管膜瘤的转移方式为沿手术通道向皮切口附近颅外转移，而髓母细胞瘤则随脑脊液向脑或脊髓及脑室壁种植转移。%Objective To investigate the metastatic way of intracranialtumors.Method Five cased of glioma were resected and recurred in distant locations.The radiological characteristics（CT MRI），the properties seem during the operation and the pathological result of the tumors are analyzed.Results Two cases of ependymoma developed metastasis subcutaneously near the incision.Three cases of medulloblastoma recurred in side ventricle wall，frontal，temporal lobe and thoracic segment of spinal cord respectively.Conclusion Met astasis may occur in glioma.Extrscranial metastasis to cutaneous incision through surgical tract mainy ocurrs in ependymoma，while adjacent intracerebral metastasis occurs in medulloblastoma，the tumor cells are often planted in spinal cord or ventricle wall through the cerebrospinal fluid flow.
