靶控输注依托咪酯镇静意识消失的半数有效血浆靶浓度

摘要

Objective To determine the half-effective target plasma concentration(Cp50) of etomidate for lose of consciousness and a fifty percent decrease of bispectral index(BIS). Methods Target-controlled infusion(TCl) of etomidate in different concentrtions ( 100,150,200,250,300,350,500 and 700 ng/ml) was performed in 20 ASA Ⅰ or Ⅱ patients without premedication. Infusion of each TCI concentration was maintained at least 8 min. Sedation level was evaluated with OAA/S and BIS. OAA/S ≤ 3 was defined as unconsciousness. Results The Cp50 and Cp95 for lose of consciousness were (337±11) ng/ml and (397 ± 13) ng/ml, respectively. The Cp50 was (551 ±17)ng/ml for a fifty percent decrease of BIS. Conclusion The Cp50 of TCI etomidate for lose of consciousness and a fifty percent decrease of BIS is determined in the present group of Chinese patients.%目的 确定国人脑电双频指数(BIS)降低50%和50%患者意识消失所需要的靶控输注依托咪酯的半数有效血浆靶浓度(Cp50).方法 不用术前药的20例ASA I～Ⅱ级患者接受预先设定的依托咪酯靶控输注浓度100～700 ng/ml,监测BIS,并以镇静/警觉评分(OAA/S评分)评估镇静程度,OAA/S≤3认为意识消失.结果 BIS降低50%时的Cp50为(551士17)ng/ml;意识消失的Cp50和Cp95分别是(337士11)ng/ml和(397士13)ng/ml.结论 该研究确定了本组国人患者BIS降低50%和50%患者意识消失所需要依托咪酯的Cp50.
