丙泊酚对大鼠肝脏I-R损伤性胰岛β细胞功能和外周组织胰岛素敏感性下降的影响

         

摘要

目的 观察丙泊酚对大鼠肝脏缺血-再灌注(I-R)损伤性胰岛β细胞分泌功能和外周组织胰岛素敏感性下降的影响.方法 40只SD大鼠随机均分为丙泊酚组(P组)和I-R组.建立70%肝脏缺血1h、再灌注12h大鼠模型后,P组从肝门阻断前20 min泵入丙泊酚20mg·kg-1 ·h-1直至再灌注2h,IR组泵入等量生理盐水.再灌注12h后每组取10只采血,测定血糖、胰岛素浓度,并计算胰岛素分泌指数;每组另取10只行高血糖钳夹实验,测定血胰岛素浓度,计算葡萄糖代谢率和胰岛素敏感性指数.结果 与I-R组相比,P组血糖和胰岛素浓度降低[(11.65±1.02) mmol/L vs.(6.23±3.67) mmol/L和(116.45±10.87) mU/L vs.(90.24±5.68) mU/L](P<0.05或P<0.01),胰岛素分泌指数升高(473.75±46.24 vs.857.63±20.56)(P<0.01).高血糖钳夹实验显示,与I-R组相比,P组血胰岛素浓度、胰岛素敏感性指数、葡萄糖代谢率升高[(69.05±13.10) mU/L vs.(84.27±15.04) mU/L、32.33±7.02 vs.58.31±7.69、(29.62±7.43) mg· kg-1· min-1 vs.(56.09±10.56) mg· kg-1 ·min-1](P<0.05或P<0.01).结论 丙泊酚能缓解大鼠肝脏I-R损伤导致的胰岛β细胞分泌功能和外周组织胰岛素敏感性的下降.%Objective To observe the effects of propofol on decreased secretion of pancreatic beta cells and the insulin sensitivity caused by ischemia-reperf usion( I-R) injury in rats. Methods The hepatic I-R injury model was established in 40 SD rats by 70% hepatic ischemia for 1h and reperfusion for 12 h. The rats were equally randomized into two groups of P (treated with propofol via jugular vein from 20 min before ischemia to 120 min after reperf usion) and I-R (treated with equal volume of normal saline). The concentrations of blood glucose and plasma insulin were determined and insulin secretion index was calculated at 12 h after reperfusion in 10 rats of each group. Hyperglycemia clamp test was performed at 12 h after reperfusion in other 10 rats of each group. The concentration of plasma insulin was determined and insulin sensitivity index and glucose metabolism rate were calculated. Results Compared with group I-R, the concentrations of blood glucose and plasma insulin were drcreased [(11. 65±1. 02) mmol/L vs. (6. 23 ± 3. 67) mmol/L and (116. 45±10. 87) mU/L vs. (90.24±5.68) mU/L, respectively] (P<0. 05 or P<0. 01), while insulin secretion index was increased(473. 75±46. 24 vs. 857. 63 ±20. 56)(P<0. 01) in group P. The hyperglycemia clamp test showed that the concentration of plasma insulin, insulin sensitivity index and glucose metabolism rate in group P were higher than those in group I-R [(84. 27 ± 15. 04) mU/L vs. (69. 05 ± 13. 10) mU/L, 58.31 ± 7.69 vs. 32.33 ± 7.02 and (56.09 ± 10.56) mg · kg-1 · min-1 vs. (29.62 ± 7.43) mg·kg-1·min-1, respectively] ( P<0. 05 or P<0. 01). Conclusion Propofol can attenuate the decreases of insulin secretion and sensitivity caused by hepatic I-R injury in rats.
