SWE技术应用于移植肾检查的成像质量影响因素分析

摘要

目的 应用超声剪切波弹性成像(shear wave elastography,SWE)技术研究肾移植术后肾功能稳定患者的声像图特征,探讨影响SWE成像质量的因素.方法 对272例肾移植术后肾功能稳定的患者行常规超声及剪切波弹性成像检查,根据所观测皮髓质区域超声弹性成像彩色充盈情况,将图像质量评分为0分(成像差),1分(成像一般),2分(成像良好),3分(成像佳),分析年龄、身体质量指数、移植肾体积、肾脏血流、肾皮质厚度、皮下脂肪层厚度、肌肉层厚度与图像质量的关系.结果 272例患者中,SWE成像质量0分共17例,1分49例,2分共120例,3分共86例.身体质量指数、移植肾体积、皮下脂肪层厚度、肌肉层厚度、肾皮质厚度与图像质量均有关.结论 可应用SWE技术进行移植肾检查及定量分析.应用SWE进行移植肾检查时,应考虑成像质量影响因素,在样本量充足的情况下进行分层实验设计.%Objective To explore the impact factors of image quality of shear wave elastography (SWE) for patients with stable renal function after renal transplantation.Methods Two-dimensional ultrasound,CDFI and SWE were performed in 272 patients with stable renal function after renal transplantation.According to the color filling of area-of-interest when playing SWE,we defined the image quality as bad(0 score),inadequate(1 score),good(2 score),and excellent(3 score).The correlations of image quality with sex,age,body mass index (BMI),volume of kidney,flow of kidney artery,thickness of kidney parenchyma,and thickness of the layer of muscle and fat between renal transplantation and skin were analyzed.Results Among the 272 cases,17 cases' score of SWE image quality was 0,49 cases' 1,120 cases' 2,and 86 cases' 3.The image quality correlated with body mass index,kidney volume,renal cortex thickness,thicknesses of muscle and fat layers between the renal transplantation and skin.Conclusions SWE can be used in renal transplantation and quantitative analysis.When using SWE in renal transplantation and the samples are sufficient,we should take these impact factors into consideration for better layered experimental design.
