早期及进展期胃癌患者生存素与存活期的相关性分析

摘要

Objective To explore the correlation between Survivin and survival time in patients with early and progression gastric carcinoma.Methods the data of gastric-mucosa tissue biopsy in 22 patients with progression gastric carcinoma and 16 patients with early gastric carcinoma were collected and the surviving levels of the gastric mucosas were detected by immunohistochemical method.The survival time of the patients were observed through a 5-year follow-up and analyzed through Kaplan-Meier method.The correlation between survivin and survival time was analyzed by Spearman method.Results Survivin in early gastric carcinoma was obviously lower than that of progression ones,and their difference was statistically significant(Z=-2.137,P=0.033),The accumulative survival rates of one,three and five year in early gastric carcinoma patients were 100.0%,63.6% and 23.4%,respectively,and mean of survival time was 42 months;Whereas the accumulative survival rates of one,three and five year was 87.5%,22.5%,0.0%,respectively and the mean of survival time was 24 months in progression gastric carcinoma patients.The difference of two groups was statistically significant(χ2=14.144,P=0.000).Survivin and survival time showed negative correlation(C=-0.367,P=0.023)Conclusion The contents of survivin were closely related to the different stage of gastric carcinoma,and there was a negative correlation between survivin and survival time,which needs large sample and multicentre study to confirm the result.%目的 探讨不同分期胃癌患者生存素(survivin)的含量与及生存期长短是否具有相关性.方法 分别收集22例初次确诊早期胃癌患者及16例进展期胃癌患者的胃粘膜活检组织,用免疫组化法检测胃粘膜生存素的含量;对以上患者进行为期5年的生存期观察.分别用Kaplan-Meier进行生存分析,用Spearman法分析survivin含量与生存期长短的相关性.结果 早期胃癌患者survivin表达强度明显低于进展期患者(Z=-2.137,P=0.033).早期胃癌患者的1年、3年及5年累计生存率分别为100.0%、63.6%及23.4%,中位生存期为42个月;进展期胃癌患者生存率分别为87.5%、22.5%及0.0%,中位生存期为24个月;两组的生存率差异具有统计学意义(χ2=14.144,P=0.000);38例患者的survivin表达强度与生存时间呈负相关性(相关系数C=0.367,P=0.023).结论 Survivin的表达强度与胃癌的分期有关,并且与存活期的呈负相关性,但该结论 有待大样本的研究进一步验证.
