PiCCO技术在感染性休克早期目标导向治疗的临床指导价值

摘要

Objective To explore the clinical application of PiCCO for EGDT in septic shock.Methods 60 patients were randomly divided into control group(n=30)and therapy group (n=30).The patients in the therapy group were treated with EGDT under the guidance of PiCCO technology,the patients in the control group were accepted routine treatment with cycle and capacity.Results The therapy group achieved EGDT time shorter than the control group,MAP、CVP,ScvO2,PaO2/FiO2 significantly higher than the control group,The infusion was significantly higher than the control group in 6h,24 h,The vasoactive drugs(dopamine)dose was significantly lower than the control group,the treatment group with multiple organ dysfunction syndrome (MODS) and mortality was significantly lower than the control group,the difference was significant (P<0.05).Conclusion Application PiCCO technical director in EGDT,it can guide the recovery of tissue perfusion indicators,it is clinical guidance with good value in the comprehensive treatment of septic shock..%目的 探讨PiCCO(动脉轮廓法)技术在感染性休克的早期导向治疗(EGDT)中的临床指导价值.方法 将60例患者分成对照组和治疗组各30例,治疗组采用PiCCO技术指导下进行EGDT,对照组采用循环与容量支持等常规治疗.结果 治疗组达到EGDT目标所需时间短于对照组,治疗组的MAP、CVP、ScvO2、PaO2/FiO2显著高于对照组,治疗组患者6 h、24h输液量较对照组增多,血管活性药物(多巴胺)用量低于对照组,同时治疗组多脏器功能障碍综合征(MODS)发生率及病死率低于对照组,差异具有统计学意义(p<0.05).结论 应用PiCCO技术指导下进行EGDT,可以利用组织灌注指标指导复苏,在感染性休克综合治疗中具有良好的临床指导价值.
