膀胱癌组织及尿脱落细胞组织中微小染色体维持蛋白2的检测

摘要

Objective To investigate the expression and clinical significance of minichromosome maintenance protein 2 ( MCM2 ) in bladder carcinoma tissue and urine exfoliate cells of patients with bladder cancer.Methods 25 patients with bladder cancer,40 patients with other urological diseases,and 10 healthy volunteers were enrolled; MCM2 in bladder carcinoma tissue and urine exfoliate cells was detected with RT-PCR.At the same time,urinary cytology and the expression of MCM2 in bladder carcinoma were conducted.Results MCM2 positive rate in bladder carcinoma tissue was 96％.MCM2 expression had a intense correlation with differentiation degree,depth of invasion,lymph node metastasis,and recurrence hazard.The positive rate of MCM2 in urine exfoliate ceils in patients with bladder cancer increased with the invasion,lymph node metastasis,and recurrence hazard.There were significant differences among different differentiation degree and depth of invasion.The sensitivity of MCM2 ( 96％ ) and negative of predict value( 97.6％ ) for diagnosing bladder cancer was signifieantly higher than those( 52％,76.8％ ) of cytology ( P ＜ 0.05 ).Conclusion MCM2 can reflect the reproductive activity of the cell of bladder carcinoma.RT-PCR is a sensitive and specific method to detect MCM2 in urine exfoliate cells in bladder cancer.%目的 探讨膀胱癌组织和尿脱落细胞组织中微小染色体维持蛋白2(minichromosome maintenance2 protein,MCM2)的表达及意义.方法 采用RT-PCR方法检测25例膀胱癌患者、28例泌尿系良性疾病患者和15例正常对照组尿脱落细胞组织中MCM2的表达；并对膀胱癌组织MCM2的表达进行检测,同时行尿细胞学检查.结果 膀胱癌组织标本中MCM2阳性率为96％ (24/25),MCM2表达量与分化程度、浸润深度、淋巴结转移、是否复发密切相关(P＜0.05).膀胱癌患者尿脱落细胞中MCM2阳性率随分化程度和浸润浓度及复发的增加而逐渐增加；且在不同分化程度及不同浸润浓度、是否复发之间的差异也有显著性(P＜0.05).尿脱落细胞中MCM2检测诊断膀胱癌的灵敏度(96％)、阴性预测值(97.6％)明显高于尿细胞学检查(52％、76.8％),差异有显著性(p＜0.05).结论 MCM2能很好地反映膀胱癌组织中细胞的增殖能力,RT-PCR方法检测膀胱癌组织及尿脱落细胞组织中MCM2表达是一种敏感、特异的膀胱癌诊断方法.
