某地区HPV基因型与宫颈病毒感染的分析

             

摘要

目的 探讨人乳头瘤病毒(HPV)基因型感染与宫颈病变的关系.方法 对该院门诊就诊的1 336例育龄女性进行13种高危型HPV检测,对其中182例阳性者,通过快速导流杂交芯片进行21种HPV基因型检测.结果 检出13种HPV基因型,其中检出单一感染42例,占45.1%,混合型感染140例,占54.9%.HPV16型检出率最高,为52例(28.6%);HPV 33型检出29例(15.9%);HPV58型检出26例(14.3%);HPV 52型检出26例(14.3%);HPV68型检出13例(7.1%).还在混合感染中发现低危型CP8304型和6型分别为4例和2例.在11例浸润宫颈癌中,发现感染HPV16型有7例(混合型感染3例,单一感染4例),感染HPV 33型3例(混合型感染2例,单一感染1例).在11例子宫颈上皮内瘤样病变Ⅲ(CINⅢ)患者中,HPV16型感染9例(混合型感染3例,单一感染6例),HPV33型感染2例(混合型感染),HPV 31型感染1例(单一感染).结论 HPV分型检测可以分析不同地区HPV感染的流行状况,同时根据此结果可以针对性的开发适用于此地区的HPV病毒基因预防性疫苗.%Objective To investigate the relationship between human papilloma virus (HPV) genotypes infection and cervical diseases .Methods 1 336 cases of females at reproductive age were enrolled and detected for 13 kinds of high risk HPV . Rapid flow-through hybridization chip was utilized for the detection of 21 kinds of HPV genotype in 182 cases of subjects positive with high risk HPV .Results In all subjects positive with HPV , the proportion of single infection and mixed infection were 45 .1% (42/ 182) and 54 .9% (140/182). The detection rates of HPV 16 , 33 , 58 , 52,68 were 28 .6% ,15.9% (29/182),14 .3% (26/182),14. 3% (26/182) and 7 .1% (13/182) . Low risk HPV CP8304 and HPV6 were of 4 and 2 cases in mixed infection . In 11 cases of patients with infiltrative uterine cervix cancer , 7 cases were infected by HPV 16 , including 3 for mixed infection and 4 for single infection , 3 cases were infected by HPV33 , including 2 for mixed infection and 1 for single infection . In 11 cases of patients with cervical intraepithelial neoplasia(CIN Ⅲ ),9 cases were infected by HPV 16 , including 3 for mixed infection and 6 for single infection , 2 cases were with mixed infection of HPV33 and 1 case was with single infection of HPV31 .Conclusion Detection of HPV genotypes could be used to understand the prevalence situation of HPV infection and further to supply reference for the research and de -velopment of HPV vaccine fit for different regions .
