Objective To investigate the epidemiological features, complications, treatment and pathogenic characteristics of severe cases of hand, foot and mouth disease (HFMD) in Yuxi City during 2010-2011. Methods Descriptive epidemiology was used to analyze age, gender, geographical and onset, time distribution of severe HFMD cases, and the data involving the diagnosis and treatment were analyzed as well. Fluorescent reverse transcription-ploy me rase chain reaction was used for laboratory detection. Results Of 330 severe HFMD cases, children under the age of 3 accounted for the largest proportion (86,36%). These severe cases were widely distributed in the counties and districts of Yuxi city, and more cases were found in Hongta district, Ershan county and Yirnen county. HFMD mostly occurred from May to October, The severe HFMD cases were mainly among diaspora children and rural children. Lip to 36.06% of the severe HPMD cases selected private clinics for the first diagnosis, and the confirmed diagnosis rate was 42.86%:. The severe HFMD cases developed the complications of various systems. Laboratory findings showed that severe cases were mainly infected by enterovirus 71. Conclusions Severe cases occurring in HFMD epidemics in Yuxi City during 2010-2011 are widely distributed with high incidence. The differences in onstc time, geographical and population distribution are significant. HFMD has become a major infectious disease in Yuxi city, and efforts in the prevention, diagnosis and treatment of HFMD in primary health care institutions should be strengthened.%目的 分析玉溪市2010-2011 年重症手足口病的流行特征、并发症、治疗和病原学特点.方法 采用描述流行病学方法,对重症手足口病患者的年龄、性别、地区分布、发病季节等进行分析,同时对诊断和治疗等方面的资料进行总结、分析;采用荧光反转录聚合酶链反应法进行实验室检测.结果 330 例重症手足口病中,3 岁以下患儿居多,占总发病数的86.36%;各县区均有发生,其中红塔区、峨山县和易门县为高发区;发病高峰在每年5-10 月;发病人群多为散居儿童和农村儿童;36.06%的重症患者首次就诊于个体诊所,首次确诊率为42.86%;重症患者出现各系统并发症;实验室结果揭示重症病例以肠道病毒EV71 型感染为主.结论 玉溪市2010-2011 年重症手足口病呈高发、散发态势,存在时间、地区和人群差异,手足口病已成为玉溪市主要传染病,应加强基层医疗单位的预防、诊断和治疗水平.
