ICU综合征发生的相关因素分析及干预对策

摘要

Objective To understand the current situation of ICU syndrome in patients of the intensive care unit,and explore the related influence factors of ICU syndrome, and propose the corresponding intervention strategy.Methods Three instruments were used to collect data of 280 cases of pa-tients who received treatment in ICU from the First Affiliated Hospital of Zhengzhou University,including the demographic information,ICU admission and treatment information,the Acute Physiology and Chronic Health Evaluation systemⅡ(A-PACHEⅡ).Results The incidence of ICU syndrome was 24.6%.Logistic regression showed that influence factors of ICU syndrome were age (OR:2.051;95% CI:1.021 -4. 119),family per capita income (OR:0.444;95% CI:0.256 -0.770),medical treatment charge of payment (OR:0.386;95% CI:0.203 -0.733 ),APACHEII scores (OR:2.384;95% CI:1.420 -4.002);operation time (OR:1.572;95% CI:1. 149 -2. 151),ICU admission time (OR:4. 168;95% CI:2.559 -6.788),sputum suction (OR:0. 196;95% CI:0.040 -0.954).Conclusions The incidence of ICU syndrome is high.Age,family per capita income,medical treatment charge of payment,APACHEII scores,operation time,ICU admission time,sputum suction were the influence factors of ICU syndrome.Adopt targeted nursing intervention measures and encourage family members to participate in psychological nursing can be helpful for reduc-ing the incidence of ICU syndrome.%目的：了解重症监护室患者发生ICU综合征的现状，探讨ICU综合征发生的相关影响因素，并提出相应的干预对策。方法分别用自行设计的一般情况问卷、入住ICU治疗情况调查表、急性生理及慢性健康状况评分系统（APACHEⅡ）对入住郑州大学第一附属医院ICU进行治疗的280例患者进行调查研究。结果 ICU综合征发生率为24.6％。Logistic回归结果显示，年龄（OR：2.051；95％ CI：1.021～4.119）、家庭人均月收入（OR：0.444；95％ CI：0.256～0.770）、医疗费用支付方式（OR：0.386；95％ CI：0.203～0.733）、、APACHEⅡ评分（OR：2.384；95％ CI：1.420～4.002）、手术时间（OR：1.572；95％CI：1.149～2.151）、入住ICU时间（OR：4.168；95％ CI：2.559～6.788）、吸痰治疗（OR：0.196；95％ CI：0.040～0.954）是ICU综合征发生的独立危险因素。结论 ICU综合征发生率较高，受多种因素影响。患者的年龄、家庭人均月收入、医疗费用支付方式、APACHEII评分（疾病严重程度）、手术时间、入住ICU时间、是否曾吸痰是ICU综合征的主要影响因素。针对性地采取护理干预措施，并鼓励患者家属参与到心理护理中，有助于降低ICU综合征的发生率。
