肾癌患者血清VHL HIF-1αHIF-2α 水平变化及意义

         

摘要

目的:探讨血清希佩尔-林道基因(VHL)、缺氧诱导因子(HIF)1α及HIF-2α在肾癌患者中的变化及意义.方法:收集2015年1月至2016年4月的45例肾癌患者作为肾癌组,并在同一时期收集健康查体者作为对照组,检测两组血清VHL、HIF-1α及HIF-2α水平;对肾癌患者的癌灶最大直径进行测定;对肾癌患者进行6个月随访,观察并记录复发患者情况.结果:肾癌组比对照组血清VHL水平明显下降,但HIF-1α 和HIF-2α 水平显著上升(P<0.01);肾癌患者血清VHL、HIF-1α 和HIF-2α表达和患者年龄、性别没有明显关系(P>0.05),但和TNM分期、临床分期、癌灶直径以及淋巴结转移呈显著相关(P<0.01).未复发组比较,复发组病例术后血清VHL水平显著更低,而HIF-1α和HIF-2α水平明显更高(P<0.05,P<0.01).结论:血清VHL、HIF-1α及HIF-2α水平与肾癌病理分级、临床分期、癌灶大小以及淋巴结转移相关,可能对肾透明细胞癌的发生和发展有重要作用.%Objective:To research the changes and significance of serum von hippel-lindau ( VHL ) , hypoxia-inducible factor ( HIF)-1α and HIF-2α in patients with renal cancer. Method:45 cases of renal carcinoma were collected from January 2015 to April 2016 as the renal cell cancer group, and at the same time the collection of healthy persons as control group, serum VHL, HIF-1αand HIF-2αlevel of two groups were examined;The maximum diameter of renal cancer patients was measured; Patients with renal cell carcinoma were followed up for 6 months, observed and recorded the recurrence patients. Results:The level of serum VHL in renal cancer group decreased significantly compared with that in control group, while levels of HIF-1α and HIF-2αwere obviously higher( P<0.01) . Expression of serum VHL, HIF-1αand HIF-2αwas not related to age and sex in renal carcinoma ( P>0. 05) ,while there was significantly correlated with pathological grade, clinical stages, diameter of cancer site and lym-phatic metastasis ( P<0.01) . Level of serum VHL in recurrent cases was obviously lower, while levels of HIF-1αand HIF-2αwere obviously higher than non-re-current cases( P< 0.05,P<0.01) . Conclusion:Levels of serum VHL, HIF-1α and HIF-2α are correlated significantly with pathological grade, clinical stages, size of carcinoma and lymphatic metastasis, which indi-cates indexes mentioned above could be play an important role in the occurrence and development of clear cell renal carcinoma.
