Objective To discuss the value of transvaginal ultrasound diagnosis for cornual pregnancy.Methods 70 cornual pregnancies are randomly divided into two groups: the experimental group which was examined by transvaginal ultrasound and the control group which was examined by conventional transabdominal sonography, with 35 cases in each group. We would discuss the clinical value of the two different ultrasound diagnosis with analysis of the two groups's preoperative performance, combined with surgery and pathology results. Results The ultrasound diagnosis of the experimental group appears 30 cases correct and 5 cases wrong (One case of intrauterine pregnancy was misdiagnosed as cornual pregnancy, two cases of interstitial pregnancy were misdiagnosed as cornual pregnancy, and another two cases were not positioned), and its ultrasound accuracy rates of 85.71％ (30/35); The diagnosis of the control group appears 20 cases correct and 15 cases wong. 10 of them were clearly diagnosed after use of transvaginal ultrasound diagnosis. and its ultrasound accuracy rates of 57.41％ (20/3 5). The diagnostic accuracy of two statistics have a very significant difference (X2=7.00, P＜0.05). Conclusion Transvaginal ultrasound imaging has higher diagnostic value to cornual pregnancy. The patients with clinical diagnosis of pregnancy, but conventional abdominal ultrasound examination can not obviously tip embryo implantation position and suspected ectopic pregnancy,should be routinely examined by transvaginal ultrasound.%目的 探讨经阴道超声检查诊断宫角妊娠的价值.方法 将宫角妊娠患者随机分为实验组和对照组,以经阴道超声组为实验组,以常规经腹超声组为对照组,两组病例各35例,分析两组术前经腹部和经阴道超声检查表现,并结合手术及病理结果探讨两种途径超声诊断的临床应用价值.结果 实验组超声诊断正确30例,误诊5例(1例宫内妊娠误诊为宫角妊娠,2例输卵管间质部妊娠误诊为宫角妊娠,未予定位2例),超声检查准确率85.71%(30/35);对照组诊断正确20例,误诊15例,其中10例改用经阴道超声后明确诊断,超声检查准确率57.14%(20/35).两组诊断准确率经统计学处理,差异有统计学意义(x<'2>=7.00,P<0.05).结论 经阴道超声显像对宫角妊娠具有较高的诊断价值,对于临床拟诊妊娠病例,常规经腹部超声检查无法明显提示胚胎着床位置、怀疑异位妊娠者应常规行此检查,以期为临床提供可靠依据.
