自体游离股薄肌腱移植治疗Kuwade Ⅲ型陈旧性跟腱断裂

摘要

目的 探讨自体游离股薄肌腱移植治疗Kuwade Ⅲ型陈旧性跟腱断裂的临床疗效.方法 2009年6 月至2011 年12 月,自体游离股薄肌腱移植治疗Kuwade Ⅲ型陈旧性跟腱断裂18 例,术后给予石膏外固定,常规康复治疗,观察伤口愈合情况,测量小腿周径和小腿三头肌跖屈肌力,Amer-Lindholm 标准评定踝关节的功能情况.结果 所有18例患者均获得随访,平均随访时间15个月(7~20个月).其中2例患者出现切口并发症,经换药后治愈,其余16例患者伤口一期愈合;所有18例患者患肢小腿的最大周径均小于对侧肢体,小腿三头肌肌力弱于对侧肢体,患肢踝关节的活动度与对侧相比,轻度受限,Amer-Lindholm标准评分6例为优,12例为良.结论 自体游离股薄肌腱移植治疗陈旧性跟腱断裂的临床疗效确定,值得推广应用.%Objective To evaluate the clinical efficacy of free gracilis tendon graft for the reconstruct of Kuwade III type chronic tears of the achilles tendon. Methods From June 2009 to December 2011, 18 patients with chronic achilles tendon rupture of Kuwade M type were treated with free gracilis tendon. Plaster immobilization and routine rehabilitation therapy were performed postoperatively. The efficacy was evaluated by Amer-Lindholm criterion. Results All patients were followed-up for 7 to 20 months (mean 15 months). Evaluated by Amer-lindholm criterion, the results were excellent in 6 cases, good in 12 cases and no worse case. Conclusion Free gracilis tendon was safe and perfect s to repair chronic Achilles tendon rupture of Kuwade III type.
