盐酸右美托咪定复合全身麻醉对阑尾切除术后患者认知功能的影响

Objective To study the effect of dexmedetomidine hydrochloride combined with general anesthesia intervention on cognitive function of appendectomy patients after operation.Method 80 cases of appendectomy operation patients as the research object, the patients were divided into observation group and control group. Observation group: before induction of anesthesia, intravenous drip Dexmedetomidine Hydrochloride injection. Control group: intravenous infusion of normal saline. Anaesthesia was induced with propofol, fentanyl and vecuronium. After anesthesia with intravenous patient-controlled analgesia.Results HR,SBP, DBP, two groups of patients in operation lower than that before operation. After the operation higher than that of before operation. Among them, the changes of the observation group better than the control group, with significant differences among the three groups (P<0.05). 6h after operation, 12, 24 and 48 h, VAS score and Ramsay sedation score, the observation group were better than the control group, the difference was statistically significant (P<0.05).Conclusion Application of dexmedetomidine hydrochloride as appendectomy assisted drug operation under general anesthesia, can effectively meet the pain, sedation. Reduce the stress response in patients with reduced rate, careful, blood pressure fluctuation. At the same time with less side effects, the occurrence characteristics of the low rate of adverse reactions of patients. The reason may be related to dexmedetomidine anti-inflammatory effects.%目的：探讨盐酸右美托咪定复合全身麻醉对阑尾切除术后患者认知功能的影响。方法选择80例行阑尾切除术患者为研究对象，将患者分为观察组和对照组。观察组在麻醉诱导前静脉滴注盐酸右美托咪定注射液，对照组静脉滴注等量生理盐水。麻醉的诱导均采用丙泊酚、芬太尼和维库溴铵，麻醉后均采用静脉自控镇痛。结果两组患者术中的HR、SBP、DBP均较术前降低，术后均较术前升高。其中，观察组的变化幅度较对照组小，组间比较具有统计学意义（P＜0.05）。结论应用盐酸右美托咪定作为阑尾切除术全身麻醉的辅助用药，可以有效地满足镇痛、镇静作用，降低患者的应激反应，减小心率、血压的波动，同时具有副作用少，患者发生不良反应率低的特点，其原因可能与右美托咪定的抗炎作用有关。
