系统性红斑狼疮患者幸福感指数影响因素分析及心理治疗探讨

摘要

Objective To study the influential factors for Index of Well-Being of SLE patients and its psychotherapy pattern. Methods A total of 46 SLE patients,aged between 1 5 and 58,who were treated in out-patient of Endocrine Immune Rheumatism Center of Dongguan KangHua Hospital from June 1 ,201 2 to May 31 ,201 3.These patients were investigated with questionnaires of Egma Minnen av Bardndosnauppforstran,Family Assessment Device,Defense Styles questionnaire,Index of Well-Being,Index of General Affect,and Systemic Lupus Erythematosus Disease Activity Index.The results were analyzed with multiple stepwise regression analysis to study the influential factors for Index of Well-Being of SLE patients. Results Index of Well-Being had positive relation with Undoing,had negative relation with Somatization,and had positive relation with Fantasy. Conclusion Undoing,somatization and fantasy play very important roles in SLE patients'Index of Well-Being;we could treat those influential factors with individuality psychotherapy,family psychotherapy,group psychotherapy,or marital family-based psychotherapy.%目的：了解影响系统性红斑狼疮患者幸福感指数的影响因素，并探讨心理治疗模式。方法采用父母教养方式量表、家庭功能评定量表、防御方式问卷、幸福指数量表及狼疮活动指数量表，对2012年6月1日-2013年5月31日就诊于东莞康华医院内分泌及风湿免疫中心的46名15～58岁的系统性红斑狼疮患者进行问卷调查。采用多元逐步回归对幸福感指数进行因素分析。结果幸福感指数总分与解除机制（t ＝3.015，P ＝0.005＜0.05）呈正相关，与躯体化机制（t ＝-2.815，P ＝0.008＜0.05）呈负相关；与幻想机制（t ＝1.804，P ＝0.079＜0.1）呈正相关。结论系统性红斑狼疮患者人群解除、躯体化、幻想机制对狼疮患者的幸福感指数有着重要的作用；针对这些影响因素的干预方式有个体心理治疗、家庭心理治疗、团体心理治疗和婚恋家庭取向的心理治疗。
