目的：测定分析不同类型过敏性紫癜（HSP）患儿外周静脉血中B细胞激活因子（BAFF）、免疫球蛋白与补体的水平，研究HSP免疫发病机制中BAFF可能发挥的作用。方法收集不同类型HSP患儿，包括皮肤型患儿35例、关节型33例、腹型30例、肾型38例、混合型31例，均为初诊活动期，同时设立对照组。免疫球蛋白和补体用速率散射比浊法测定； BAFF蛋白含量用酶联免疫吸附试验（ELISA）方法检测。结果各型患儿BAFF水平均高于对照组；混合型HSP患儿IgA增高、补体C3降低，其余各型免疫球蛋白与补体差异无统计学意义；混合型HSP患儿血清中BAFF与IgA表达水平呈正相关性，与C3表达水平呈负相关性。结论 HSP活动期患儿外周血中BAFF表达增高，可以导致体液免疫功能上调，表现为免疫球蛋白IgA的水平升高与补体C3的降低。%Objective To analysis the levels of B cell-activating factor (BAFF), immunoglobulin and comple-ments in children with different subtypes of Henoch-schonlein purpura (HSP), and to explore the possible role of BAFF in the pathogenesis of HSP. Methods Children with a primary diagnosis of active HSP, including cases of skin type (n=35), joint type (n=33), abdominal type (n=30), kidney type (n=38) and mixed type (n=31), were included in the study. A group of normal children were selected as the control group. Rate scatter nephelometry was used to de-tect the levels of immunoglobulin and complements, and BAFF were examined with ELISA. Results Higher serum level of BAFF was found in each subtype of HSP compared with the control group. Children with mixed type HSP had higher level of IgA and lowered level of C3 compared with the controls, whereas this difference was not observed in other subtypes of HSP. The serum BAFF level in the mixed type HSP was positively correlated with IgA level and negatively correlated with C3 level. Conclusion The increased BAFF expression in peripheral blood of active HSP can enhance the humoral immune function, as reflected by a rise in IgA and reduction in C3.
