超声检查对大肠肿瘤早期诊断基础与临床应用系列研究——临床应用

摘要

Five hundred and twenty one cases were examined by abdominal US during UBCE and endorectal US.The detective rate of moderate and advance stage cancer was l00%.The coincidenee rate of US cornpared with surgery and pathology was 95.4%.The accurate rate of localization by US was 96%.The smallest bowel cancer detected with 5MHz transducer was 1.4×0.8cm.Ultrasonic mass suervey of large bowel diseases,showed:organic diseases aceounted for 9.6%of high rick persons.The coincidence rate of US diagnosis compared with pathological diagnsis waS 81.6%,Bowel cancers were diagnosed in two of them.Ultrasonic survey during UBCE can be performed in basic units.It is a safe,simple and effective screening for early detection of colorectal cancel.%大肠超声显像液灌注(UBCE)腹部与直肠腔内超声检查结直肠良恶性肿瘤与普查共521例,经手术、纤维肠镜活检病理诊断及气钡双重造影与钡灌肠证实,超声对中晚期癌肿的发现率100%,与病理符合95.4%,定位准确性96%;早期肠癌仅2.9%.结直肠息肉<1.0cm检出率74%,1.1～2cm96%.频率5MHz检出最小息肉0.6cm×0.4cm,肠癌0.8cm×1.4cm.按癌肿浸润肠壁程度超声分期与病理符合率75.2%.详述了结直肠癌,息肉,良性肿瘤,肠壁间病变的超声图像与鉴别方法.大肠病变超声普查发现器质性病变4.9%;占高危人群的9.6%与病理符合81.6%,其中2例大肠癌;并发现血吸虫病大肠的超声表现.对沿海农渔村及血吸虫流行区普查,证实UBCE超声检查不受条件限制,可广泛用于基层,是早期诊断:大肠肿瘤简便、有效的过筛方法,有实际应用价值.
