目的 探讨围生期巨细胞病毒感染对免疫性血小板减少性紫癜 (ITP) 患儿临床症状、治疗效果、预后等的影响,并讨论其相应的综合护理模式.方法 选取本院收治的非病毒感染所致ITP患儿50例、围生期巨细胞病毒感染导致ITP患儿82例.非病毒感染所致ITP患儿设定为对照组,接受常规治疗和护理; 围生期巨细胞病毒感染导致ITP患儿为实验组,均增加抗病毒治疗,随机分为两组,一组接受常规护理,另一组接受ITP综合护理.比较三组患儿临床症状的轻重及治疗效果.结果 对照组出血事件的发生率低于实验组,程度较实验组轻,但组间不存在统计学差异,对照组治疗的有效率和治愈率均显著高于实验组,(P < 0. 05).实验组中,接受ITP综合护理的患儿治疗效果优于接受常规护理的患儿,但差异不具有显著性.结论 围生期巨细胞病毒感染引起的儿童ITP临床症状、治疗效果和预后较非病毒引起ITP差,ITP综合护理能在一定程度上减轻患者的出血情况并提高治疗效果.%Objective To investigate the effect of perinatal cytomegalovirus infection on immune thrombocytopenic purpura(ITP)in children and detect its comprehensive nursing. Methods 50 children with ITP caused by other reasons who were set as control group and gave routine treatment and nursing were emrolled. While 82 children with ITP caused by cytomegalovirus infection were set as test group and randomly divided into two groups,one group were given daliy nursing while another goup were given ITP-pointed nursing. Antiviral therapy were both added. The clinical symptoms and outcomes were compared between the three groups. Results Compared with the control group,the clinical symptoms and outcomes were worse in the test group. In the meantime,the efficiency rate and cure rate were significantly lower(P < 0. 05). In the test group,the outcomes of children reciving comprehensive nursing were much better than those reciving routine nursing. Conclusion The clinical symptoms and outcomes are poor in children with ITP caused by cytomegalovirus infection than other reasons,ITP- pointed comprehensive nursing can extenuate clinical symptoms and improve the treatment outcome.
