动态实时测定生长激素在垂体生长激素腺瘤手术治疗中的意义

摘要

Objective To evaluate the significance of the intraoperative growth hormone (GH) measurement in assessment of surgical remission and prognosis for GH-secreting pituitary adenomas.Methods Clinical data of 106 patients undergoing transsphenoidal surgery for pituitary adenoma were analyzed retrospectively.The traditional GH measurement was performed in 34 patients (TGHM group) and intraoperative GH measurement in 72 (IGHM group).Results Total remission was achieved in 60 patients (56.60％),and the remission rate of IGHM group was obviously higher than TGHM group (P ＜ 0.001).There was also a statistically significant difference in remission rate of macroadenoma,noninvasive adenoma and invasive adenoma between the two groups (P ＜ 0.05),but there was no obvious difference in remission rate of microadenoma between the two groups (P ＞ 0.05).Conclusions Intraoperative real-time GH measurement can improve the surgical efficacy and provide important prediction for prognosis,thus deserving wider clinical application.%目的探讨术中生长激素(GH)水平在垂体GH腺瘤缓解及预后判断中的意义.方法回顾性分析106例经鼻蝶垂体腺瘤切除术的GH腺瘤病人的临床资料,其中采用常规GH测量方法34例(常规GH测量组),术中GH测量72例(术中GH测量组).结果本组术后共缓解60例(56.60％),术中GH测量组缓解率明显高于常规GH测量组(P＜0.001).两组间大腺瘤、非侵袭性腺瘤和侵袭性腺瘤的缓解率差异均有统计学意义(P＜0.05),而两组微腺瘤的缓解率差异无统计学意义(P＞0.05).结论术中实时GH测定可提高手术疗效,对预后有重要预测价值,值得临床推广.
