右美托咪定辅助下瑞芬太尼呼吸抑制EC50与高强度聚焦超声热剂量的关系研究

     

摘要

Objective: To explore the relationship of dexmedetomidine assisted remifentanil respiratory inhibition of EC50 with high intensity focused ultrasound thermal dose.Methods:84 patients were treated with HIFU in our hospital patients,according to the different HIFU treatment for A thermal dose,B,C,D four groups,21 cases in each group,each group will continue in the dexmedetomidine infusion,TCI of remifentanil,and according to the sequential administration of different doses.Results:In A group,remifentanil respiratory inhibition of EC50 and 95% conifdence interval was 3.84(3.38~4.22)ng/ml,B group of remifentanil respiratory inhibition of EC50 and 95% conifdence interval was 3.99(3.43~4.57)ng/ml,C group of remifentanil respiratory inhibition of EC50 and 95% conifdence interval was 3.88(3.41~4.36) ng/ml,D group of remifentanil respiratory inhibition of EC50 and 95% conifdence interval 4.08 (3.56~4.62) ng/ml.Conclusion:In the dexmedetomidine assisted,can reduce the HIFU with different thermal dose of remifentanil respiratory inhibition,EC50 value,the rational use of compatibility,and provide scientiifc basis for the treatment of HIFU patients in anesthesia is more safe,comfortable.%目的:探索右美托咪定辅助下瑞芬太尼呼吸抑制EC50与高强度聚焦超声热剂量的关系。方法:选取于我院进行HIFU治疗的肿瘤患者84例,根据HIFU治疗热剂量的不同分为A、B、C、D四组,每组21例,每组在右美托咪定持续输注下,靶控输注瑞芬太尼,并按序贯给药法予以不同剂量。结果:A组瑞芬太尼呼吸抑制EC50及95%可信区间为3.84(3.38~4.22)ng/ml,B组瑞芬太尼呼吸抑制EC50及95%可信区间为3.99(3.43~4.57)ng/ml,C组瑞芬太尼呼吸抑制EC50及95%可信区间为3.88(3.41~4.36)ng/ml,D组瑞芬太尼呼吸抑制EC50及95%可信区间为4.08(3.56~4.62)ng/ml。结论:在右美托咪定辅助下,可降低不同热剂量的HIFU治疗中,瑞芬太尼呼吸抑制的EC50值,将两者合理配伍使用,为HIFU治疗中患者更加安全、舒适的麻醉方法提供科学依据。
