(Department of Anesthesiology, Beijing Millennium Monument Hospital, Affiliated Capital Medical University,Beijing 100038, China)[ABSTRACT]Objective: To observe the clinical application value of remifentanil infusion combined with propofol target controlled infusion in children with short operation.Methods: 112 cases of children were randomly divided into control group (56 cases) and observation group (56 cases). The control group was treated with seven sevolfurane inhalation anesthesia, the observation group received intravenous infusion of remifentanil and propofol.Results:The two groups of T1 T3 to T5 HR, RR, map and t0 time difference (P<0.05); observation group T2 ~ T4 HR, RR, map was significantly higher than that of control group (P<0.05). The recovery time, pull tube time, returned to the wards time of the observation group was significantly shorter than that in control group (P<0.05), patient satisfaction and stops the medicine after 10min conscious level was significantly higher than the control group (P<0.05); after the drug was stopped 15min groups alertness without significant difference (P>0.05). In the operation of the restlessness, nausea and vomiting, choking cough, dyspnea, throat discomfort and other adverse reaction rate, the observation group was significantly lower than that in the control group (P<0.05).Conclusion: In pediatric surgery, combined with propofol target controled infusion of propofol target controled infusion can stabilize the hemodynamic parameters, accelerate the recovery process, shorten the recovery time, reduce adverse reactions.%目的:观察瑞芬太尼静脉输注联合丙泊酚靶控输注在小儿短小手术中的临床应用价值.方法:行短小手术小儿112例,随机分为对照组(56例)和观察组(56例),对照组行七氟烷吸入麻醉,观察组行瑞芬太尼静脉输注联合丙泊酚靶控输注麻醉,观察两组患者血流动力学、麻醉效果、麻醉不良反应、满意度等指标.结果:⑴两组T1,T3~T5 HR、RR、MAP与T0时刻差异明显(P<0.05);观察组T2~T4 HR、RR、MAP明显高于与对照组(P<0.05).⑵苏醒时间、拔管时间、送返病房时间观察组明显短于对照组(P<0.05),患者满意度及停药后10min清醒程度明显高于对照组(P<0.05);停药后15min两组清醒程度无明显差异(P>0.05).⑶术中躁动、恶心呕吐、呛咳、呼吸困难、咽部不适及其他不良反应发生率,观察组明显低于对照组(P<0.05).结论:在小儿短小手术中,瑞芬太尼静脉输注联合丙泊酚靶控输注可稳定患儿血流动力学指标,加快清醒过程,缩短苏醒时间,减少不良反应发生.
