10000例妇科门诊阴道分泌物检测结果分析

摘要

目的：通过对首都医科大学附属北京妇产医院妇科门诊患者阴道分泌物清洁度、霉菌、滴虫、细菌性阴道病的快速检测结果进行分析，探讨常见阴道病的发病特点，提高临床诊断率。方法对收集到的10000例妇科患者标本分别进行显微镜检查和细菌性阴道病诊断试剂盒检测。结果霉菌性、细菌性、滴虫性三种阴道病发病率依次为28．6％、11．5％、3．67％。三种阴道病阳性率随着阴道分泌物清洁度等级的增加而增加，Ⅲ度和Ⅳ度清洁度组较 I 度和Ⅱ度清洁度组，组间比较差异具有显著性（χ2＝40．90-2245．37，P ＜0．01），有统计学意义。除＜20岁组和20-40岁组细菌性阴道病外，其余各年龄组霉菌、细菌性阴道病、总阳性率组间比较差异均具有显著性（χ2＝6．85-589．37，P ＜0.05），有统计学意义。40-60岁年龄组霉菌性、细菌性阴道病的阳性率最高，其次是20-40岁年龄组、＜20岁年龄组、≥60岁年龄组；滴虫性阴道病各年龄组的阳性率差异不大，无统计学意义。结论本院妇科患者霉菌性和细菌性阴道病发病率较高，三种阴道病的阳性率与阴道分泌物清洁度及患者年龄（滴虫性阴道病除外）有密切关系。联合检测阴道分泌物清洁度、霉菌、滴虫、细菌性阴道病对阴道病的诊治有重要意义。%Objective Base on the results of analysis of vaginal discharges in patients from gynecologic outpatient department,and aim of this study was to discover the pathogenic character of vaginosis.Methods A statistical analysis was conducted after testing samples of 10000 patients from gynecologic outpatient department in terms of fungi,tricho-monad,cleaning degree and bacterial vaginosis by microscope and bacterial vaginosis diagnostic kit.Results Fungi, trichomonad,bacterial vaginosis,three kinds of vaginal disease rates were 28.6%,11.5%,3.67%.Positive rate of three vaginal disease increases with cleaning degree level increases,The differences between III、IV of the cleaning degree groups and Ⅰ、Ⅱ of the cleaning degree groups are statistically significant (χ2 =40.90-2245.37,P <0.01).In addition to the <20 age group and the 20-40 age group of bacterial vaginosis,differences among the rest of the age groups are statistically significant(χ2 = 6.85-589.37,P <0.05).Positive rate of 40-60 age group of fungal,bacterial vaginosis is the highest,followed by the 20-40 age group,<20 age group,≥60 group;positive rate of all age groups of Trichomon-ad vaginosis is no statistical significant.Conclusion Fungi and bacterial vaginosis of gynecological patients in our hospi-tal have a higher prevalence,positive rate of three vaginosis have close relationship with cleaning degree and patient age (except Trichomonad vaginosis).Combined detection for cleaning degree,Fungi,Trichomonad,bacterial vaginosis is very important for diagnosis and treatment of vaginosis.
