过敏性紫癜患儿肾损害易感性及免疫因素临床分析

ObjectiveTo explore the susceptibility and immune factors of Henoch-Schonlein purpura children with renal damage.MethodFrom January 2013 to September 2014, 150 children with Henoch-Schonlein purpura in our hospital were selected as the research objects, and they were divided into renal damage group (71 cases) and non-renal damage group (79 cases). Levels of T lymphocyte subpopulation, Cys-c, IgA, IgG, IgM, LTB4 between the two groups were compared. The susceptible factors of renal damage in children with Henoch-Schonlein purpura were analyzed by single factor method.ResultLevels of CD3+, CD4+ and CD4+/CD8+ ratio in renal damage group were significantly lower than non-renal damage group (P<0.05), CD8+ level was signiifcantly higher than non-renal damage group (P<0.05). Levels of Cys-c, IgA, LTB4, TNF-α in renal damage group were signiifcantly higher than non-renal damage group (P<0.05);and levels of IgG, IgM, IL-21 between the two groups were not signiifcantly different (P>0.05). The rash occurred repeatedly for more than 1 month (OR=2.327) was susceptible factor to Henoch-Schonlein purpura children with renal damage.ConclusionThere is signiifcant differences T lymphocyte subsets, Cys-c, IgA, LTB4, TNF-α of Henoch-Schonlein purpura children with or without renal damage. Single factor analysis shows that the rash occurred repeatedly for more than 1 month (OR=2.327) is susceptible factor to Henoch-Schonlein purpura children with renal damage.%目的：探讨过敏性紫癜患儿发生肾损害的易感因素及相关免疫因素。方法选取2013年1月至2014年9月于本院住院治疗的150例过敏性紫癜患儿为研究对象，根据患儿是否发生肾损害分为肾损害组（71例）和非肾损害组（79例），比较两组患儿外周血T淋巴细胞亚群、胱抑素C（Cys-c）、血清免疫球蛋白（IgA、IgG、IgM）、白三烯B4（LTB4）水平，并采用单因素法分析过敏性紫癜患儿发生肾损害的易感因素。结果肾损害组患儿CD3+、CD4+水平及CD4+/CD8+比值均显著低于非肾损害组（P＜0.05），CD8+水平显著高于非肾损害组（P＜0.05）。肾损害组患儿Cys-c、IgA、LTB4、肿瘤坏死因子-α（TNF-α）水平均显著高于非肾损害组（P＜0.05）；两组患儿IgG、IgM、IL-21水平比较差异均无显著性（P＞0.05）。皮疹反复发生＞1个月（OR＝2.327）是过敏性紫癜患儿发生肾损害的易感因素。结论发生肾损害与非肾损害的过敏性紫癜患儿T淋巴细胞亚群、Cys-c、IgA、LTB4、TNF-α等存在显著差异，单因素分析显示皮疹反复发生＞1个月是并发肾损害的易感因素。
