下咽部鳞状细胞癌患者人乳头瘤病毒感染及p53蛋白表达的检测分析

摘要

目的 了解下咽部鳞状细胞癌患者人乳头瘤病毒(human papillomavirus,HPV)感染及p53蛋白表达的情况.方法 对收集到的15例临床诊断下咽部鳞状细胞癌病例的石蜡组织标本应用Luminex及PCR的方法对HPV感染进行基因检测,应用PCR方法对p53基因进行扩增、测序和分析,应用免疫组织化学的方法对HPV16/18E6蛋白及p53蛋白进行检测.结果 在15例临床诊断的下咽癌病例中,通过Luminex及PCR的方法发现6例病例具有HPV病毒的感染;通过免疫组织化学的方法发现2例病例具有HPV16/18的感染,总阳性率达到46.7％(7/15);有9例患者p53蛋白表达为阳性(60％,9/15),另外有9例患者p53基因出现突变(40％,6/15).结论 此研究探讨了下咽癌患者中HPV病毒的感染率与p53蛋白的表达及突变情况,为进一步阐明下咽癌的发病机制奠定基础.%Objective To analyze the infection of human papillomavirus and expression of p53 in patients with hypopharynx squamous cell carcinoma. Methods The pathological samples of 15 clinically diagnosed hypopharynx cancer patients were collected. Lunimex and PCR techniques were used to detect the human papillomavirus (HPV) gene and immunohistochemical method was used to analyze the HPV protein expression in the samples. PCR and direct sequencing were used to screen the mutations in p53 gene and immunohistochemistry was used to evaluate the expressing profiles of p53 proteins. Results In the 15 cases, 6 were positive for HPV16 / 18 gene and 2 cases were positive for HPV16 / 18 E6 protein expression. The total positive rate was about 46. 7％ (7 / 15). In addition, 9 of 15 (60. 0％ ) cases had sequencing-confirmed p53 mutations and p53 immunohistochemistry (IHC) confirmed 9 (60. 0％ ) cases had positive staining. Conclusions The high HPV infection rate and p53 expression in hypopharynx squamous cell carcinoma patients in the study lays a foundation for further elucidating the pathogenesis of this kind of cancer.
