中性粒细胞-淋巴细胞比值与冠状动脉慢血流关系的Meta分析

     

摘要

Objective To review systematically the relationship between neutrophil-lymphocyte ratio (NLR) and coronary slow flow (CSF).Methods The databases of PubMed, CNKI, WanFang Database and VIP Database were retrieved for collecting the case-control studies on the relationship between NLR and CSF from the database establishment time to Nov. 30, 2016. All the studies were screened, data was extracted and checked crossly, and quality was reviewed by two researchers independently according to inclusion and exclusion criteria, and the final included studies were given a Meta-analysis by using RevMan5.2 software.Results There were totally 6 studies included involving 851 cases, among them 426 in CSF group and 425 in control group. The results of Meta-analysis showed that the level of white blood cell count (WBC) was higher in CSF than that in control group (MD=0.59, 95%CI: 0.35~0.83,P<0.001,I2=0%). The level of neutrophil (Neu) was higher in CSF group than that in control group (MD=0.65, 95%CI: 0.28~1.01,P<0.001,I2=64%). The quantity of lymphocytes (Lym) was lower in CSF group than that in control group (MD=-0.21, 95%CI: -0.4097~ -0.0023,P=0.05,I2=71%). The level of NLR was much higher in CSF group (MD=0.57, 95%CI: 0.41~0.73,P<0.001,I2=47%).Conclusion The levels of WBC, Neu, Lym and NLR were higher in CSF group, which indicates that inflammatory reaction is an important mechanism of CSF.%目的 系统评价中性粒细胞-淋巴细胞比值(NLR)与冠状动脉慢血流(CSF)的关系.方法 检索PubMed、中国知网、万方、维普等中英文数据库,收集关于探究中性粒细胞-淋巴细胞比值与冠状动脉慢血流关系的病例对照研究,检索时间均为建库时间至2016年11月30日.按设定的纳入及排除标准由2名评价者独立选择试验提取资料交叉核对并进行文献质量评估,对最终纳入的研究文献采用RevMan5.2软件进行Meta评价.结果 纳入6篇文献,共有851例研究对象,其中CSF组426例,对照组425例.Meta分析结果 表明,相比于对照组,白细胞(WBC)水平在CSF组水平更高,差异具有统计学意义(MD=0.59,95%CI:0.35~0.83,P<0.001,I2=0%);CSF组中性粒细胞(Neu)水平高于对照组,其差异具有统计学意义(MD=0.65,95%CI:0.28~1.01,P<0.001,I2=64%);CSF组患者中其淋巴细胞(Lym)数值低于对照组,其结果 具有统计学意义(MD=-0.21,95%CI:-0.4097~-0.0023,P=0.05,I2=71%);NLR水平在CSF组水平更高(MD=0.57,95%CI:0.41~0.73,P<0.001,I2=47%),结果 具有统计学意义.结论 WBC、Neu、Lym及NLR水平在CSF组中较高,提示炎症反应为CSF的重要作用机制.
