慢性乙型肝炎患者外周循环DNA的定量检测

             

摘要

Objective To quantitative analyze peripheral circulating DNA and evaluate its potential clinical value on diagnosis of liver damage. Methods Circulating DNA level of patients with chronic hepatitis B were quantified with a novel duplex real time PCR assay. Results The level of circulating DNA in control group, mild, moderate and severe chronic hepatitis B was( 21 ± 9,29 ± 8,38 ± 8 and 81 ± 16 ) μg/L, respectively. As the condition of patients with chronic hepatitis B exacerbated, circulating DNA level increased gradually. Circulating DNA level of patients with severe chronic hepatitis B was significantly higher than that of control group( P < 0.01 ). There was a positive relationship among circulating DNA level, alanine aminotransferase and aspartate aminotransferase( r= 0. 612 and 0. 632 respectively, P < 0.01 ). Circulating DNA level of serum HBeAg positive group was significantly higher than that of serum HBeAg negative group( 68 μg/L ± 14 μg/L vs 23 μg/L ± 12 μg/L, P < 0.05 ). Conclusion Circulating DNA level may reflect liver inflammation degree and may become a marker of liver inflammation activity.%目的 探讨外周循环DNA水平定量检测在诊断肝组织损伤中的价值.方法 纳入慢性乙型肝炎轻度、中度、重度患者及健康对照者各30例,采用双重荧光定量PCR法检测血浆中循环DNA的含量.结果 健康对照组,慢性乙型肝炎轻度、中度和重度患者外周循环DNA水平分别为(21±9、29±8、38±8和81±16 )μg/L,随着慢性乙型肝炎患者病情加重,外周循环DNA水平逐渐升高,慢性乙型肝炎重度患者外周循环DNA水平显著高于健康对照组(P<0.01).慢性乙型肝炎患者外周循环DNA水平与ALT和AST呈显著正相关(r值分别为0.612和0.632,P值均<0.01).HBeAg阳性慢性乙型肝炎患者外周循环DNA显著高于HBeAg阴性组(68 μg/L±14 μg/L vs 23 μg/L±12 μg/L,P<0.05).结论 外周循环DNA的含量可反映肝脏的炎性反应程度,有可能作为肝脏炎性反应活动程度的一项监测指标.
