龈沟液及血清中Th17、Treg相关细胞因子在慢性牙周炎中的作用

摘要

目的:探讨Th17、Treg相关细胞因子在慢性牙周炎中的作用.方法:选择轻、中重度慢性牙周炎患者120例以及牙周健康者60例,分为正常对照组、轻度牙周炎组、中重度牙周炎组(n=60).比较各组患者的探诊深度(PD)、菌斑指数(PLI)、出血指数(BI)、附着丧失(AL);ELISA法检测各组患者血清及龈沟液中白介素-17(IL-17)、白介素-23(IL-23)、白介素-10(IL-10)、转化生长因子-β(TGF-β)的水平.Pearson直线相关分析法对血清及龈沟液中相关细胞因子与牙周临床指标进行相关性分析.结果:慢性牙周炎患者各项牙周指标(PD、PLI、BI、AL)相比,轻度组、中重度组明显高于对照组(P<0.05);中重度组明显高于轻度组(P<0.05).龈沟液及血清中IL-17、IL-23水平轻度组、中重度组均明显高于对照组(P<0.05),中重度组明显高于轻度组(P<0.05).龈沟液及血清中IL-10、TGF-β水平在轻度组最高,中重度组最低(P<0.05).血清及龈沟液中IL-17、IL-23水平与PD、AL、PLI、BI值均呈显著正相关.结论:慢性牙周炎患者存在Th17/Treg相关细胞因子失衡现象;IL-17、IL-23与慢性牙周炎发生发展关系密切.%AIM:To explore the role of Th17 and Treg related cytokines in patients with chronic periodonti-tis. METHODS:180 subjects were divided into normal control group, mild chronic periodontitis group and severe chronic periodontitis group(n=60).Probing depth(PD),plaque index (PLI),bleeding index (BI),clinical attach-ment loss (AL) in all subjects were examened.The levels of IL-17,IL-23,IL-10 and TGF-β in serum and gingival crevicular fluid(GCF) were measured by ELISA.The correlation of cytokines in serum and GCF with clinical indexes was statistically analyzed. RESULTS:PD, AL, PLI and BI were higher in patients with chronic periodontitis than those in control group(P<0.05);in severe group were higher than in the mild group(P<0.05).The serum IL-17, IL-23 levels were higher in chronic periodontitis patients than those in normal control group(P<0.05), in severe group were higher than in mild group(P<0.05).The levels of IL-10 and TGF-β in serum and GCF were the highest in the mild group,and the lowest in the severe group(P<0.05).Correlation analysis showed that the levels of IL-17 and IL-23 in serum and GCF were positively correlated with PD, AL, PLI and BI values respectively. CONCLU-SIOIN:There is an imbalance of Th17/Treg associated cytokines in patients with chronic periodontitis. IL-17 and IL-23 are closely related to chronic periodontitis.
