兔膝骨性关节炎软骨MMP-2表达动态变化与MRI表现的对照研究

             

摘要

目的:通过动态监测兔膝关节骨性关节炎(OA)模型软骨内基质金属蛋白酶-2 (MMP-2)表达水平的变化,并与MRI分级对照分析,探讨MRI软骨损伤分级与MMP-2表达水平之间的内在关系.方法:健康新西兰大白兔35只,随机分为5组,除对照组外其他组均采用前交叉韧带切断术建立OA模型,分别在术后2周、4周、6周、8周行MRI检查和病理学观察,观察退变软骨的病理变化和MMP-2表达水平.结果:随着时间延长,软骨破坏日趋明显,各组间MRI分级差异均有统计学意义(P<0.05).MMP-2表达水平在术后第4周最高,对照组MMP-2表达水平与手术组相比差异均有统计学意义(P<0.05),术后4周组高于术后8周组(P<0.05).各组软骨损伤的MRI分级与MMP-2表达存在相关性(Spearman's相关系数,r=0.547,P=0.001).结论:MRI能够准确评价不同时期OA软骨退变的病理改变;MMP-2在退变的早期和中期起重要作用.%Objective: To investigate the relationship between matrix metalloproteinase-2(MMP-2)in degeneration cartilage and the MRI grading of cartilage in rabbit' s knee osteoarthritis. Methods: Thirty-five healthy New Zealand rabbits were divided into five groups randomly, including control group, 2-weeks group, 4-weeks group, 6-weeks group and 8-weeks group. Anterior cruciate ligament transaction was used to build OA model. MRI and pathological observation were carried out at corresponding time point. Results: The damage of cartilage became more severely with time. The MRI grades were different from each other a-mong all groups( P <0.05). The expression level of MMP-2 reached peak in the fourth week. The expression level of MMP-2 in control group was lower than those of operational groups(P <0. 05). The expression of MMP-2 in 4-weeks group was higher than that of 8-weeks group( P <0. 05),while no differences were found among the other operational groups( P >0. 05). A correlation between MRI grading and MMP-2 expression were found using Spearman's rank test (Correlation coefficient r = 0. 547, P=0. 001). Conclusion: The pathological scores and MRI grading gradually increase with postoperative time. MMP-2 plays a part in the prophase and the metaphase of the degeneration. MRI is able to reflect the degree of cartilage injury directly.
