上皮钙黏蛋白和黏附分子CD44V6及基质金属蛋白酶2在贲门癌中的表达及意义

摘要

目的 探讨上皮钙黏蛋白(E-cad)、黏附分子CD44V6、基质金属蛋白酶2(MMP-2)在贲门癌组织中的表达水平及其临床意义.方法 采用免疫组织化学方法检测64例贲门癌组织和20例癌旁组织中E-cad、CD44V6、MMP-2蛋白表达水平.结果 E-cad、CD44V6、MMP-2在贲门癌组织中的阳性表达率分别为20.6%、60.9%和64.1%;E-cad在癌旁组织中的阳性表达率为85.7%,而CD44V6、MMP-2在癌旁组织中基本不表达.贲门癌组织中E-cad的阳性表达率显著低于癌旁组织,而CD44V6、MMP-2的阳性表达率则显著高于癌旁组织,差异有统计学意义(P＜0.05).肿瘤细胞分化程度不同,患者癌组织中E-cad、CD44V6、MMP-2阳性表达率比较,差异均有统计学意义(P＜0.05);淋巴结未转移者癌组织中E-cad、CD44V6、MMP-2阳性表达率(分别为20.7%、37.9%和48.3%)与淋巴结转移者(分别为2.9%、80.0%和77.1%)比较,差异均有统计学意义(P＜0.05).结论 E-cad、CD44V6、MMP-2可能参与了贲门癌的发生发展,检测三者的表达水平有助于判断转移及预后.%Objective To explore the expression and significance of E - cad, CD44V6 and MMP - 2 in gastric cardia carcinoma tissues. Methods Expression levels of E - cad, CD44V6 and MMP - 2 proteins in 64 samples of gastric cardia carcinoma tissues and 20 samples of adjacent tissues were detected using immunohistochemistry. Results Positive expression rates of E - cad, CD44V6 and MMP -2 in the tissues of gastric cardia carcinoma were 20. 6% , 60. 9% and 64. 1% respectively; positive expression rate of E - cad in the adjacent tissues was 85. 7% ; CD44V6 and MMP -2 in the adjacent tissues were almost negatively expressed. Positive expression rate of E - cad in the gastric cardia carcinoma tissues was significantly lower than that in the adjacent tissues, while positive expression rates of CD44V6 and MMP - 2 in the gastric cardia carcinoma tissues were significantly higher than those in the adjacent tissues with significant differences ( P <0. 05 ) . Positive expression rates of E - cad, CD44V6 and MMP - 2 were correlated to tumor cell differentiation degrees and lymph node metastases. Positive expression rates of E - cad, CD44V6 and MMP - 2 were 20. 7% , 37. 9% and 48. 3% respectively in the 29 cases with lymph node metastases and were 2. 9% , 80. 0% and 77. 1% respectively in the 35 cases without lymph node metastases. Conclusion E - cad, CD44V6 and MMP - 2 protein may be involved in the development of gastric cardia carcinoma and thus might be useful in predicting the metastasis and prognosis of gastric cardia carcinoma.
