含缓释碳水化合物高单不饱和脂肪酸配方肠内营养制剂对2型糖尿病的影响研究

摘要

Objective To discuss the influence of enteral nutrition formula rich in slow - release carbohydrates and mono - unsaturated fatty acids and traditional fiber type powder formula on levels of blood glucose and insulin of patients wTith type 2 diabetes mellitus ( T2DM ). Methods 91 T2DM patients were randomly divided into three groups: control group ( 30 cases ), experimental group 1 ( 30 cases ) and experimental group 2 ( 31 cases ). The three groups were all given diabetic diet in the first three days. On the fourth day, the control group, experimental group 1 and experimental group 2 were given diabetic diet , fiber type powder and slow - release enteral nutrition formula with the same calorie respectively as their breakfasts. During the fifth day to the seventh day, the three groups were all given diabetic diet. On the eighth day, the control group, experimental group 1 and experimental group 2 were given diabetic diet, slow - release enteral nutrition formula and fiber type powder with the same calorie respectively. Fasting blood glucose ( FBG ), postprandial blood glucose ( PBG ), fasting insulin ( FINS ), postprandial 2h insulin and the area under blood glucose concentration curve ( AUCG ) on the fourth day and eighth day were compared among the three formulas. Results The FBG and FINS of the three formulas showed no statistically significant differences ( P >0. 05 ). The postprandial 2 h blood glucose, postprandial 2 h insulin and AUCG of the slow - release enteral nutrition formula were all significantly lower than those of fiber type enteral nutrition powder and diabetic diet ( P <0. 05 ). Conclusion Compared with fiber type enteral nutrition powder and diabetic diet, slow - release enteral nutrition formula can effectively reduce the postprandial blood glucose and insulin response level of T2DM patients, and thus it is more suitable for patients with diabetes.%目的 探讨含缓释碳水化合物高单不饱和脂肪酸配方肠内营养制剂(缓释配方)与传统的糖尿病专用的纤维型粉剂配方对2型糖尿病(T2DM)患者血糖和胰岛素水平的影响.方法 将91例T2DM患者随机分为3组:对照组(30例)、试验组1(30例)、试验组2(31例).试验前3 d内,3组均给予糖尿病餐;试验第4天,对照组、试验组1和试验组2分别给予相同热量的糖尿病餐、纤维型粉剂和缓释配方作为早餐食用;试验5~7 d,3组均给予糖尿病餐;试验第8天,对照组、试验组1和试验组2分别给予相同热量的糖尿病餐、缓释配方、纤维型粉剂.分析并比较试验第4天和第8天3种配方空腹及餐后血糖、胰岛素、血糖曲线下面积(AUCG)的均值.结果 3种配方空腹血糖及空腹胰岛素水平比较,差异均无统计学意义(P>0.05);缓释配方餐后2 h血糖及胰岛素水平、AUCG均低于糖尿病餐和纤维型粉剂,差异有统计学意义(P<0.05).结论 缓释配方与纤维型粉剂及糖尿病餐比较,能显著降低T2DM患者餐后血糖和胰岛素的应答水平,更适合糖尿病患者使用.
