同型半胱氨酸、糖化血红蛋白和尿微量白蛋白联合检测对早期糖尿病肾损伤的诊断价值

         

摘要

Objective To investigate the value of homocysteine, glycosylated hemoglobin and urine albumin combined detection in diagnosis of early diabetic renal injury. Methods All 40 patients with early renal disease in our hospital,data were analyzed, set the experiment group and 40 patients in our hospital at the same time were selected as the healthy control group, used cyclic enzymatic detection Hcy, ion exchange liquid chromatography HbA1c, the rate of scattering assay used U-mAlb, comparing two groups of homocysteine, gly cosylated hemoglobin and urine albumin and other indicators. Results The serum Hcy levels(23.71±1.91)μmol/L,HbA1c levels(10.01±2.53)%,U-mAlb level(263.34±6.23)mg/L in experiment group was significantly higher than the control group(P<0.05);Hcy abnormal blood test group was 70%, HbA1c anomaly detection rate of 60%, U-mAlb level anomaly detection rate of 57.5%, signifi cantly higher (blood Hcy, HbA1c and urine abnormality U-mAlb detection rate was 0%)(P<0.05); early diabetes diagnosis using U-mAlb positive rate of 42.5%, Hcy joint U-mAlb positive rate was 70.0%, HbA1c joint U-mAlb positive rate of 67.5%, Hcy joint HbA1c joint U-mAlb positive rate was 82.5%. Conclusion Homocysteine, glycosy-lated hemoglobin and urine albumin ideal joint detection of early diagnosis of diabetic renal injury results,the positive rate of detection can be improved and should be introduced.%目的:探讨同型半胱氨酸、糖化血红蛋白和尿微量白蛋白联合检测在早期糖尿病肾病损伤患者中的临床诊断价值。方法对我院收治的40例早期糖尿病肾损伤患者资料进行分析,将其设置为实验组,选择40例同期来我院健康体检者为对照组,采用循环酶法检测Hcy,离子交换液相色谱法检测HbA1c,采用速率散射法检测U-mAIb,比较两组同型半胱氨酸、糖化血红蛋白和尿微量白蛋白等指标。结果实验组血Hcy水平为(23.71±1.91)μmol/L、HbA1c水平为(10.01±2.53)%、U-mAlb水平为(263.34±6.23)mg/L,显著高于对照组(P<0.05);实验组血Hcy异常率为70%、HbA1c异常检出率为60%,U-mAlb水平异常检出率为57.5%,显著高于对照组(血Hcy、HbA1c和尿U-mAlb的异常检出率均为0%)(P<0.05);早期糖尿病患者采用U-mAlb诊断阳性率为42.5%,Hcy联合U-mAlb阳性率为70.0%,HbA1c联合U-mAlb阳性率为67.5%,Hcy联合HbA1c联合U-mAlb阳性率为82.5%。结论同型半胱氨酸、糖化血红蛋白和尿微量白蛋白联合检测诊断早期糖尿病肾损伤效果理想,能够提高检测阳性率,值得推广使用。
