Objective To explore the influencing factors of emergency success rate of cardiopulmonary resuscitation in patients with respiratory arrest.Methods The clinical data of 245 patients who needed cardiopulmonary resuscitation were analyzed retrospectively from June 2013 to January 2016. The factors influencing the success rate of CPR emergency rescue were analyzed by univariate and Logistic multiple regression analysis.Results Of the 245 patients with CPR,47 were successful and the success rate was 19.18%.Univariate analysis showed that CPR-related factors were CPR initiation time, resuscitation site, tracheal intubation, electric shock defibrillation, adrenaline dosage and cardiogenic disease.Multivariate logistic regression analysis showed that CPR initiation time, resuscitation site, tracheal intubation, electrohardretic defibrillation, adrenaline dosage and cardiogenic disease were the main factors affecting CPR effect (OR = 7.923,10.179,9.254,9.843,8.624, 12.247,P= 0.011, 0.031, 0.000, 0.015, 0.006, 0.024). Conclusion The success rate of emergency rescue of CPR in patients with respiratory and cardiac arrest is affected by many factors.The emergency doctor should identify the patient's condition early and carry out targeted CPR rescue in the limited treatment time window to improve the success rate of first aid.%目的 探讨呼吸心跳骤停患者心肺复苏急诊抢救成功率的影响因素.方法 回顾性分析2013年6月-2016年1月海南省海口市120急救中心收治245例心肺复苏(cardiopulmonary resuscitation,CPR)患者的临床资料,采用单因素和Logistic多元回归分析对影响呼吸心跳骤停患者CPR急诊抢救成功率的影响因素进行筛选.结果245例CPR患者中,成功47例,成功率为19.18%;单因素分析结果显示,影响CPR效果的相关因素有CPR开始时间、复苏地点、气管插管、电击除颤、肾上腺素用量和心源性疾病;多因素Logistic回归分析结果显示,CPR开始时间、复苏地点、气管插管、电击除颤、肾上腺素用量和心源性疾病影响CPR效果的主要因素(OR=7.923、10.179、9.254、9.843、8.624、12.247,P=0.011、0.031、0.000、0.015、0.006、0.024).结论 呼吸心跳骤停患者CPR急诊抢救成功率受多种因素影响,急诊医生应及早识别患者的病情,并在有限的救治时间窗内进行针对性的CPR抢救,以提高急救成功率.
