颈动脉多普勒彩超在颈动脉狭窄或闭塞诊断中的应用价值

摘要

目的：探讨颈动脉多普勒彩超在颈动脉狭窄或闭塞诊断中的应用价值。方法方便选择2015年1—6月该院收治的40例疑似颈动脉狭窄或闭塞的患者为研究对象，均行颈动脉多普勒彩超(CDUS)及数字减影血管造影(DSA)检查，以DSA检查结果作为颈动脉狭窄或闭塞诊断的金标准，分析CDUS对颈动脉狭窄或闭塞诊断的敏感度、特异度、符合率。结果以阴性(正常)与阳性(狭窄或闭塞)作为切分点时，颈动脉CDUS诊断的灵敏度、特异度、诊断符合率分别为93.7%、90.7%、91.9%，与DSA诊断结果的一致性较好(Kappa=0.832)。而从临床治疗的角度考虑，以重度狭窄作为切分点，颈动脉CDUS诊断的灵敏度、特异度、诊断符合率分别为86.1%、89.5%、88.8%，与DSA诊断结果的一致性一般(Kappa=0.701)。结论颈动脉多普勒彩超对颈动脉狭窄的早期筛查具有重要应用价值。%Objective To investigate the application value of carotid stenosis or occlusion diagnosed with carotid color doppler ultrasound (CDUS). Methods Convenient choice in January to June 2015, our hospital of 40 cases of suspected pa-tients with carotid artery stenosis or occlusion as the research object,they were both given CDUS and digital subtraction an-giography (DSA) examination,based on DSA examination results as the diagnostic gold standard,analyzed the sensitivity de-gree,specific degree and coincidence rate of carotid stenosis or occlusion diagnosed with CDUS. Results With negative(nor-mal) and positive (stenosis or occlusion) as a segmentation point,the sensitivity degree,specific degree and diagnosis coinci-dence rate of carotid CDUS diagnostic were respectively 93.7%,90.7% and 91.9%,the consistency with the DSA diagnosis was good (Kappa =0.832).But considered from the angle of clinical treatment,based on severe stenosis as a segmentation point,the sensitivity degree,specific degree and diagnosis coincidence rate of carotid CDUS diagnostic were respectively 86.1%,89.5% and 88.8%,the consistency with the DSA diagnosis was general(Kappa=0.701). Conclusion Carotid CDUS has important application value of early screening of the carotid stenosis.
