CD4+CD25+CD127low调节性T细胞在结核性及恶性胸腔积液患者中的表达及意义

         

摘要

目的 探讨CD4+CD25+CD127low调节性T细胞(Treg细胞)在结核性及恶性胸腔积液患者外周血及胸腔积液中的表达及其临床意义.方法 收集初诊结核性及恶性胸腔积液患者各20例,采用流式细胞仪检测两组患者外周血及胸腔积液中CD4+CD25+CD127lowTreg细胞的表达;另外选择正常健康体检人员20例记为对照组,测定其外周血CD4+CD25+CD127lowTreg细胞的表达.结果 结核性及恶性胸腔积液患者外周血CD4+CD25+CD127low占CD4+细胞比例分别为:(7.88±2.16)%、(10.46±1.37)%,较对照组(4.74±0.74)%明显升高(P<0.05);结核性和恶性胸腔积液中CD4+CD25+CD127low占CD4+细胞比例分别为:(8.45±1.39)%、(18.98±5.24)%,均高于同组别及对照组外周血水平(P<0.05),并且恶性胸腔积液中CD4+CD25+CD127low占CD4+细胞比例明显高于结核性胸腔积液(P<0.05).结论 CD4+CD25+CD127lowTreg细胞在结核性及恶性胸腔积液患者的外周血及胸腔积液中表达比例的升高,表明其可能通过对机体免疫力的调节而参与疾病的发生发展,且其在胸腔积液中表达的高低变化可能作为结核性及恶性胸腔积液诊断的一项辅助性指标.%Objective To investigate expression of CD4+ CD25 + CD127lowTreg cells in the peripheral blood and pleural effusions of patients with tuberculosis and malignancies and explore its clinical significance. Methods Twenty samples of patients with tuberculosis and malignant pleural effusions were collected and detected the expression for CD4 + CD25 + CD127low regulator T cell by flow cytometry with 20 healthy persons as control. Results The percentages of CD4 + CD25 + CD127lowregulatory T cells in peripheral blood in patients with tuberculosis and malignant pleural effusions were 7. 88% ±2. 16% ,10. 46% ± 1. 37% ,that were much higher than the healthy group of 4. 74% ±0. 74% ( P <0. 05 ). The percentages of CD4+ CD25 + CD127lowregulatory T cells in pleural effusions in patients with tuberculosis and malignant pleural effusions were 8. 45% ± 1. 39% , 18. 98% ± 5. 24% , that were higher than the percentages in peripheral blood of the same groups and the healthy group( P < 0. 05 ). The percentages of CD 4+ CD25+ CD127low regulatory T cells of patients with malignant pleural effusions were much higher than the percentage in patients with tuberculosis. Conclusion The elevated proportion expression of CD4+ CD25 + CD127low regulatory T cells in peripheral blood and pleural effusions in patients with tuberculosis and malignant pleural effusions suggest that regulatory T cells is involved in the development of pleural effusions, and the evalua-sion of regulator T cells in pleural effusions may be used as an secondary indicator for the diagnosis of tuberculosis and malignant pleural effusions.
