目的：探讨程序性死亡配体-1（PD-L1）和程序性死亡受体-1（PD-1）在胃癌组织中的表达及其与患者临床病理特征的关系。方法收集151例行胃癌根治术患者的肿瘤组织标本及其对应的74例转移淋巴结标本，以及20例癌旁组织标本，采用免疫组织化学法检测其中 PD-L1和 PD-1蛋白表达，并分析其与患者各项临床指标关系。结果 PD-L1和PD-1在胃癌细胞及癌间质淋巴细胞表达较癌旁组织明显升高，阳性率分别为60.93%和38.41%；癌旁组织不表达 PD-L1和 PD-1；在转移淋巴结的表达与对应肿瘤组织一致，一致率分别为82.43%和87.84%。胃癌组织 PD-L1和 PD-1表达不相关。PD-L1表达与 pTNM 分期、浸润层面、淋巴结转移情况及脉管癌栓相关（P <0.05），PD-1与各临床指标不相关。结论 PD-L1和 PD-1在胃癌组织中表达显著升高，并与胃癌患者临床病理特征密切相关；PD-L1可能成为预测胃癌进展和预后的新分子标志物，阻断 PD-L1/ PD-1信号通路有望成为胃癌免疫治疗的新策略。%Objective To investigate the correlation between the expression of costimulatory molecule programmed death ligand 1(PD-L1),as well as its receptor programmed death 1(PD-1)in gastric carcinoma and clinicopatho-logic features. Methods The study comprised tumor sections from 151 gastric cancer patients treated with curative resection,74 matched lymph nodes,and 20 cases of adjacent normal tissues controls,which were evaluated for PD-L1 and PD-1 protein expression by immunohistochemistry. Results PD-L1 and PD-1 expression were increased in cancers compared to adjacent normal tissues,with positive rate of 60. 93% and 38. 41% ,respectively. Meanwhile, expressions of PD-L1 and PD-1 were highly correlated between the primary tumors and metastatic nodes,with con-cordance rates of 82. 43% and 87. 84% for PD-L1 and PD-1,respectively. PD-L1 expression in gastric carcinoma was not correlated with PD-1 expression in lymphocyte. Expression of PD-L1 was significantly correlated with some clinicopathologic features including stage,depth of invasion,lymph node metastasis and vascular metastasis(P <0. 05),but not PD-1. Conclusion PD-L1 and PD-1 are overexpressed in gastric cancer tissues. PD-L1 expres-sion plays a critical role in the pathogenesis of human gastric cancer,suggesting that it might be used as a new bio-marker to predict the disease progression and prognosis.
