The purpose of this pilot study is to describe the profile of poor medication management among community dwelling elderly using the Medication Discrepancy Tool (MDT). A quantitative, cross sectional survey design was used for this study. The study population consisted of 30 participants aged 65 years and older who resided in the community. The assessment was focused on medications patient was consuming at home against the discharge medication list. Findings from the study identified 40% of patients experienced one or more medication discrepancies. Living arrangement (p=0.045) and number of co-morbidities (p=0.014) significantly predicts number of discrepancies. This study has identified the high risk groups who are more susceptible in experiencing medication discrepancies upon discharge. Results from this study can help identify possible solutions to ameliorate this concern.
