Radiocarpal dislocations: four cases in multitrauma patients. Early results.

Radiocarpal dislocations are rare and severe lesions and high velocity accident is the main cause. Four multitrauma patients with associated radiocarpal dislocation were admitted in our department, between May 2008 and July 2009. Radial styloid fracture was involved in 3 cases; one patient had open dislocation with extensor tendons ruptures. Closed reduction was done immediately in the emergency room and a splint was applied. The performed CT- 3D reconstruction demonstrated injuries' severity. Surgical reconstruction was carried out through palmar approach in two cases, dorsal in one (open dislocation), while in one patient K-W were placed percutaneously. The styloid fracture was fixed with K- W, and anchors or sutures were used to repair the ligaments. The reduction was stabilised with external fixation. After 6 weeks the K-W and external fixation were removed. Six months later the patients demonstrated carpus stability and good wrist range of motion.
