Neoplastic Disorders and Monoclonal Gammopathies

Monoclonal gammopathy of undetermined significance (MGUS) is a frequent disorder related to immunological disfunctions, ageing and several types of cancer, mainly of B-cell lines; its incidence is estimated as up to 2% in people over 50. We have studied if presence of MGUS is associated with a higher incidence of neoplastic disease. Patients and Methods: A cohort of 1,930 patients diagnosed of MGUS since 1967 was followed-up to 2000. Data source: medical records and Population Registry of Cancer of Zaragoza. Variables: age, gender, date of MGUS diagnosis, immunochemical subtype, date of cancer diagnosis;, subtype and location. Results: 1,930 patients with MGUS. Mean age: 65.5+-13.6 years, gender: 384 M/255 F. MG type: IgG 433 (67.7%), IgA 58 (9.1%), IgM 142 (22.3%), light chains disease 6 (0.9%). Incidence rate of MGUS: 13.4+-1.6 cases/10~5inh/y. Estimated cancer incidence (1977-1996) in our area: 311.24 cases/10~5mh/y. A total of 639 (33.15%) MGUS developed cancer. Estimated cancer incidence in MGUS (1977-1996): 886.9 cases/ 10~5inh/y. Risk of cancer in MGUS: 4.87. Comments: In our study, a high incidence of cancer was observed among MGUS, compared to general population. This fact must be considered as not incidental, suggesting a strong correlation between MGUS and neoplasm. This association is stronger between MGUS and haeinatological cancers.
