The present invention relates to the technical field of clinical nuclear medicine, and relates to an HER2 affibody, a diagnostic or therapeutic nuclide-labeled substance, and a preparation method for the nuclide-labeled substance and an application of the nuclide-labeled substance. The sequence of the HER2 affibody is as shown in SEQ ID NO: 1. The sequence of the HER2 affibody designed and synthesized in the present invention has good in vivo stability and pharmacokinetic properties; both the HER2 affibody and a molecular probe labeled and synthesized with a diagnostic or therapeutic radionuclide have good affinity and functional activity for HER2 molecules; and the HER2 affibody is expected to become a radiopharmaceutical for targeted HER2 imaging and tumor treatment having good application prospects.
