PPROBLEM TO BE SOLVED: To provide a technique for converting the wavefront curvature of light at high speed for a wavefront curvature modulator modulating the wavefront curvature of light by using electrooptical effect. PSOLUTION: Of the wavefront curvature modulator 22, an optical waveguide forming member 150 which forms an optical path where light travels allow to generate the electrooptical effect that the light varies in refractive index with the intensity of an electric field. The intensity of the electric field applied to the optical waveguide forming member 150 is varied to modulate the wavefront curvature of the light incident on the optical waveguide forming member 150 according to the intensity of the electric field. Further, a refractive index variation region 180 formed in the optical waveguide forming member 150 with the applied electric field is curved to a plane perpendicular to the traveling direction of the light on at least one of a side 182 where the light is incident and a side 184 where the incident light exits.
