On January 13, 2000, a 42-year-old male Fire Fighter began to feel lightheaded after performing roof ventilation, searching for fire victims in a five- story apartment building, and performing overhaul. Despite CPR by fire fighters (trained and certified as first responders) and ALS by ambulance paramedics on the scene and the hospital's ED, the victim died. NIOSH was notified of this fatality on January 16, 2001, by the United States Fire Administration. On September 6, 2001, NIOSH contacted the affected Fire Department (FD) to initiate the investigation. On December 2, 2002, a Safety and Occupational Health Specialist, an occupational physician, and an occupational nurse practitioner from the NIOSH Fire Fighter Fatality Investigation Team traveled to New York to conduct an onsite investigation of the incident.
