Evaluation of Phase I of the Medicare Health Support Pilot Program Under Traditional Fee-for-Service Medicare: 18-Month Interim Analysis, October 2008

The purpose of this Report to Congress is to report the results of RTI Internationals 18-month evaluation of eight Medicare Health Support (MHS) pilot programs implemented under Phase I of the Voluntary Chronic Care Improvement Program (CCIP) Under Traditional Fee-for-Service (FFS) Medicare pilot as authorized by Section 721 of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (Pub. L. 108173) (codified as Section 1807 of the Social Security Act, hereafter the Act). Section 721 requires the Secretary of Health and Human Services to provide for the phased-in development, testing, evaluation, and implementation of chronic care improvement programs. Prior to program implementation, the name of the initiative was changed from Chronic Care Improvement Program to Medicare Health Support, which we refer to as MHS hereafter.
