摘要

Results are presented of a project undertaken to evaluate the success of the MED-WISE Programs systems approach in effecting medication compliance in the elderly. The project had three objectives: (1) to improve the patient-physician relationship, (2) to use an educational module to increase the awareness of medication compliance in the elderly and to make them more cognizant of the importance of the pharmacist and/or physician in helping to increase their own compliance; and (3) to measure the ability of the pharmacist to become a community resource in helping the elderly to better regulate their medications. The MED-WISE Program was coordinated using the pharmacists, physicians, and community resources in Herkimer and Madison Countries. An increase in communication resulted between the patient and the physician through the use of the MED-WISE card. There was a reported increase in the number of MED-Cards that the pharmicist was seeing, and this increase correlated with increasing communication between the pharmacist and the patient.
