Effect of an ANGIOTENSIN II Analogue on ARDS

An ANGIOTENSIN II analogue (Alla), (1-Sar, 8-I1e) angiotensin II, has been used for assessing angiotensin status in hypertensive patients (1). AIIa has also been noted to increase the arterial oxygen tension (PaO2) without alteration of systemic hemodynamics in a small number of patients after trauma. THe relationship of AIIa to pulmonary function changes in patients with ARDS is uncertain. In this study, the effect of AIIa on cardiopulmonary changes associated with acute lung edema has been evaluated in an oleic acid-induced animal model of ARDS. Following oleic acid injection, pulmonary function deteriorated in all sheep. In the treated sheep, however, pulmonary funtion improved during the observation period. The increased Cardiac index (CI) and Pa02 and decreased Qs/Qt in the treated sheep suggest improved alveolar ventilation. In the systemic circulation, blood pressure was well maintained in the treated sheep. (1-Sar, 8-Ile) angiotensin II improved pulmonary function following oleic aice pulmonary edema without imparing systemic circulation. The principle drug effect improving pulmonary function appears to be on the airway.
