Neonatal liver failure is a rare but life-threatening condition, and the prognosis is generally poor.1 According to some reports, emergency liver transplantation (LT) in neonates could be of benefit. However, a major problem in liver transplantation for neonates has been the lack of size-matched donor organs. To overcome this shortage, alternatives, such as reduced-size organs, split livers, and living donors, have been used in some specialized centers. In the case of living-donor liver transplantation (LDLT) for neonates, both vascular reconstruction and graft size reduction require skilled techniques. Here we report a case of LDLT that used hyper-reduced left lateral segment (LLS) for fulminant hepatic failure (FHF) of unknown cause in a 2.8-kg infant. This patient is the second smallest LDLT recipient and the third smallest reduced left lateral segment in the literature as far as we know.
