Back in the 1990's, I managed a petrochemical plant with a pretty decent safety performance. Certainly not the worst but we had reached a plateau in terms of the statistics. Incidents were still happening, minor injuries were occurring, there was always a risk that something more serious could happen and absolutely nobody wants to be the manager who has to go and deliver bad news to someone's family.
