In April this year Juken Nissho worker Bruce Davoren climbed over a guardrail to investigate a problem with a log peeler. A fault with the clamped claws of the peeler had been identified and fixed some hours earlier, but a problem recurred and he wanted to take a closer look. After climbing over the guardrail he positioned himself within the machinery. Almost immediately the clamped claws activated, pinning him between the claw he was inspecting and the lathe machine frame. Bruce Davoren died in hospital three hours after the accident. Like many accidents, this was completely avoidable. Why did it happen?
