When taking the Hippocratic Oath, physicians promise to provide the best possible care with the minimum of harm. Although an important aim is to provide value-based care (the best possible care at the lowest necessary cost), the foremost responsibility of the clinician is to each individual patient treated. As a result, physicians are protective of their autonomy to provide treatments for each patient according to their best judgement.1 However, it is estimated that >30% of current health-care expenditures provide no or minimal clinical benefit, with substantial variation in practice and treatment intensity between providers.2 In their influential report, To err is human: building a safer health system,3 the Institute of Medicine estimated that between 44,000 and 98,000 people die in US hospitals each year as a result of medical errors. This report has had a dramatic effect on the way patients and health-care professionals view quality and safety, and has directly changed the culture of medicine.
